VERIFIED PETITION TO MODIFY BIRTH CERTIFICATE
COMES NOW (full legal name), pro se, and petitions this Honorable Court to grant a modification in Petitioner's birth certificate, and in support therefor states as follows:

ORDER TO AMEND A VITAL RECORD
This matter came before the Court on Petitioner's Verified Petition to Modify Birth Certificate, and having found that the Petitioner has already changed (his/her) name to (full legal name) via order of (name change county, order output) in Case # (name change case number), and having found that the Petitioner has undergone all medically indicated treatments, procedures, and/or surgeries to complete transition from (gender on birth certificate, male/female) to (gender desired on birth certificate), it is of the opinion of the Court that the Petitioner is entitled to the relief requested herein.
