This set of pages can help you make a Court Petition and a Court Order to change your gender marker in probate court (click those links to view the documents one at a time, and form fields in context). Fill out the fields on this page, then click the links.

You can just hit Print on this page. But when printing, set Margins to None in your browser's print settings, and disable printing headers and footers. Otherwise, the document won't print consistently. This should be the default for this page, but please double check!

Some lines may not display correctly in the print preview. They should display correctly when you print the document.

After you've printed the Petition, do not sign it yet. You will need to bring a valid photo ID and the document to a notary and sign the document in front of them. Notaries are typically available through your bank for free, or may be available through your local library for $1 per document.

After you've printed the order, do not sign anywhere on it. This document is only for the judge to sign.

Review all documents before filing with the court. We are not legal professionals. We are just some folks who have an above-average amount of experience with preparing legal documents.
