ORDER TO AMEND A VITAL RECORD
This matter came before the Court on Petitioner's Verified Petition to Modify Birth Certificate, and having found that the Petitioner has already changed (his/her) name to (full legal name) via order of (name change county, order output) in Case # (name change case number), and having found that the Petitioner has undergone all medically indicated treatments, procedures, and/or surgeries to complete transition from (gender on birth certificate, male/female) to (gender desired on birth certificate), it is of the opinion of the Court that the Petitioner is entitled to the relief requested herein. The Court hereby ORDERS, ADJUDGES, AND DECREES:
The Alabama Office of Vital Statistics shall amend the birth certificate for (full legal name), formerly known as (full name on birth certificate), born in the State of Alabama on (full date of birth), to reflect that the Petitioner's gender/sex is (gender desired on birth certificate), as provided in Ala. Code § 22-9A-19(d).
